Salary: Scale point 20 (£26,446).
Hours: 37 hours.
Location: RCT.

The post holder will work closely with GPs and allied health professionals throughout Rhondda Cynon Taf to support referred individuals to address non-medical issues that may be causing or exacerbating health problems. The post aims to support individuals to take greater control of their own health and address their needs through identifying their interests and connecting them to activities, groups and services in the community.
