The All Wales Dementia Care Pathway of Standards was published in 2021, Standard 12 specifically relates to the role of a Dementia Connector and states: ‘People living with dementia and their carers will have a named contact (connector) to offer support, advice and signposting, throughout their journey from diagnosis to end of life.’

Across Cwm Taf Morgannwg the dementia connector role is to be piloted in the three locality areas of; Merthyr/Cynon, Rhondda/Taff Ely and Bridgend. These three roles will be funded from the Regional Integration Fund (RIF) for a period of 18 months to end on the 31st of March 2025. Expressions of interest in delivering the dementia connector role across CTM are being sought.
